JOB OVERVIEW

The Designated Nursing Assistant / Certified Nursing Assistant (DNA/C.N.A) is responsible for providing direct resident care including nursing/clinical care, personal, nutritional and restorative care in the skilled nursing operations and other areas throughout the community as appropriate. Responsible for demonstrating the Mission for Sunrise Senior Living, “to champion quality of life for all seniors” in accordance with federal, state and local standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our guests/residents.

Job Description



Essential Duties

The unique responsibilities for this role include but are not limited to the essential functions listed as follows:



Resident Care Duties



  • Deliver care and services for designated group of guests/residents according to Individualized Service Plans (ISP).

  • Report all incidents immediately to appropriate supervisor / team member.

  • Report suspected abuse immediately per regulations and Sunrise standards.

  • Participate in the care planning for their designated guests/residents

  • Perform. individualized personal care and activities of daily living (ADL); documents ADL in flow sheets

  • Partner with licensed nurse to plan, develop, organize, provide and execute individualized restorative therapy programs, activities and standards for their designated guests/residents.

  • Obtain, document and report the guest/resident’s vital signs, urine output and meal intake per individualized care plan to licensed nurse as requested.

  • Observe and report any physical and/or psychosocial changes of condition requiring a licensed nurse intervention and care services.

  • Assist in maintaining assistive devices, adaptive equipment and other medical equipment to include cleaning equipment and making readily available.

  • Maintain Restorative Records for each guests/residents.

  • Maintains knowledge of federal, state/provincial and local resident care regulations



Quality Indicators



  • Strive for excellent quality care and service delivery as measured in the Quality Assurance and Performance Improvement (QAPI) process.

  • Competent of behavior interventions and management.

  • Demonstrate and knowledgeable of the following key quality improvement areas of the DNA/C.N.A. role:

  • ADL, skin care, dining assistance, bowel/bladder elimination

  • Falls and fall prevention

  • Skilled nursing transformation and Resident Centered Care model



Guest/Resident, Family and Visitor Relations



  • Respond to guest/resident, family, visitor’s inquiries, concerns in a timely and professional manner using the Problem Resolution process.

  • Invite, Encourage and Assist (IEA) guests/residents to meals.

  • Assist guests/residents when transferring or discharged by collecting belongings, escorting to transportation, other as assigned.

  • Greet new guests/residents and escort to their room; orients guests/residents to room/call lights and community layout and provide activity calendar.

  • Inventory guest/resident’s belongings and labels at time of admission and ongoing there after.

Risk Management and General Safety



  • Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.

  • Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS) and Lockout Tagout procedures.

  • Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous materials.

  • Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.

  • Ensure oxygen tanks are stored safely, exchanges guest/residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels

  • Contribute in the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.

  • Attend regular meetings; Town Hall, Department Team Meetings, Cross Over, and others as directed by the Supervisor / Department Coordinator.

  • Participate in annual in-service as set forth by federal and state/provincial regulations.

  • Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.

Experience and Qualifications


To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.



  • Maintains a valid license certificate as a Certified Nursing Assistant in accordance with federal, state/province and local regulations

  • High School Degree / GED required

  • Previous experience as a Certified Nursing Assistant preferred

  • Previous experience and desire to serve and care for seniors

  • Ability to handle multiple priorities

  • Possess written and verbal skills for effective communication and documentation

  • Competent in organizational, time management skills

  • Demonstrates good judgment, problem solving and decision making skills

  • Ability to work weekends, evenings and flexible hours, available to our customers at peak service delivery days and times

  • Ability to react and remain calm in difficult situations and be considerate

  • As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand the key essential duties for safety and regulatory compliance
